On a broad level, population health examines the health outcomes of large populations to understand what makes causes chronic conditions and how to treat them.
On the practice level, providers can study population health data sets based on geographic area, or specific conditions, to understand what it takes to improve the health and outcomes of patient populations.
Value-based care models based on patient outcomes are replacing fee-for-service payment models based on services rendered. Value-based care requires the collection, management, and analysis of large data sets.
“Having quality measures to show providers is a huge advantage. Once you have the data, you can show them what's happening,” said Brittani Anderson, Quality and Performance Manager, North Ottawa Community Health System.
